EMPLOYEES If you have experienced a work-related injury, and need medical treatment, please contact your supervisor (or the Workers’ Compensation office at 893-8050) for authorization. If you wish to see your personal physician, you must have previously completed the Designation of Personal Physician for Workers’ Compensation form and have it on file in the Workers’ Compensation Office in Business Services. Please inform the treating physician and other appropriate medical facility staff that the injury occurred while working. For further information, see the UCSB Employee’s Guide to Workers’ Compensation, or call the Workers’ Compensation office at 893-8050. SUPERVISORS During normal work hours (M-F, 8-5); please call the Workers’ Compensation office to obtain authorization for initial medical treatment for non-emergency injuries at 893-8050. If an injury occurs outside of normal work hours, contact the Workers’ Compensation office by phone in one working day. NON-EMERGENCY For serious injuries not threatening to life or limb, urgent care is available on a walk-in basis at the following facilities:
